Massage Therapy - Healing Your Skin Through Massage Therapy


The largest organ in the human body is our skin. Scientifically, skin is known as the integumentary system. We don't generally pay a lot of attention to our skin, despite its size; but it is truly a phenomenal design. An average persons skin is about 1 to 2 square meters and weighs anywhere from 9 to 11 pounds. There are three layers of skin: the epidermis, dermis and hypodermis. Massage therapy has its benefits for all three.

The outer layer of skin, which communicates directly with our environment, is the epidermis. The epidermis serves as a protective layer and aids in temperature regulation. Skin can be thick or thin, depending on where it is located. Human touch was proven to be of significant value during the war, when the mortality rate of infants receiving human touch was found to be much lower than that of infants deprived of human touch.

Going just beneath the surface we find the dermis, which is actually made up of two layers, the papillary and reticular layers. Immediately below the epidermis one finds a loosely woven mat of elastin and collagen fibers called the papillary layer. About 80% of skins thickness comes from the deep reticular layer, which is found directly beneath the papillary mat. The reticular layer is a dense layer of irregular connective tissue containing thick bundles of interlacing collagen fibers. Fibers run along many planes however most of the fibers are found running parallel to the skin's surface. The dermis' reticular layer is home to capillaries, nerve endings, touch receptors, glands, veins, arteries, and hair. Massage therapy, whether a special treat or a regular part of your life, is known to promote healthy gland activity, and aids the body in toxin removal.

The hypodermis (or superficial fascia) isn't really considered skin, but rather a subcutaneous layer of areolar and adipose tissues. Fibers extend from the dermis and anchor the skin to the hypodermis, which in turn adheres to underlying tissues and organs. The hypodermis is host to body fat and blood vessels. Generally, pressure sensitive nerve endings (pacinian corpuscles) are housed in the hypodermis, but they can sometimes be found in the reticular layer of the dermis as well. As a part of the normal aging process, the hypodermis looses some of it flexibility which can result in pain when moving. A massage therapist can manipulate the skin without the aid of tools or oil to find areas where the fascia has become inflexible or rigid. The sensitive fingers of a therapist applies pressure continuously to a region facilitating the release of the superficial fascia, thereby increasing flexibility and relieving pain.

Using Exercises For Rotator Cuff Treatment Effectively


One of the most important parts of physical therapy for the shoulder are exercises for rotator cuff muscles. This is the first treatment for shoulder problems related to injury to the rotator cuff. Stiffness, pain and weakness in the shoulder or arm are good indicators of injuries to the rotator cuff.

The R.I.C.E. treatment is used in conjunction with exercises for rotator cuff. The meaning of R.I.C.E. is rest, ice, compression and elevation. These four treatments are used to reduce swelling and inflammation that can happen due to the injury and also from rotator cuff physical therapy.

The first choice for treatment of rotator cuff injuries are exercises for rotator cuff. If physical therapy has not been effective, surgery will be recommended. This is usually a last resort.

The doctor will recommend that you avoid exercises that require you to stretch the shoulder extensively. This includes reaching backwards or turning with the injured arm. Exercises for rotator cuff begin with easy stretching that will be closely watched by a physical therapist. Strenuous exercises will not be started till some range of motion is restored. Strengthening exercises are only added to the daily workout after extensive physical therapy has been done.

Even though exercises for rotator cuff are very effective, it will come slowly in order for the proper healing to take place. If physical therapy is post-surgical, this is especially true. Rehabilitation of the shoulder muscles always includes therapy after rotator cuff surgery. Range of motion may not be completely 100%, but physical therapy is very effective in restoring the amount of use that is regained in the arm and shoulder. The physical therapist or the physician will monitor the physical therapy very closely.

Electro Therapy Method Interferential Current Therapy and Its History


Interferential Current Therapy, also known as IF, is a form of electro therapy, in which electricity is used to provide pain relief and promote tissue healing.

History
This therapy originated in Germany and was originally administered by an interferential physiotherapist. It wasn't until 1960 before the therapy was delivered by physical therapists in the U.S. It has been used by physiotherapists to reduce inflammation and swelling. It has also been observed that IFC helps sufferers of asthma and can benefit those with back pain and osteoarthritis.

Interferential Therapy and TENS
Pain treatment through mean of electricity first originated when TENS machines were developed. TENS (Transcutaneous Electronic Nerve Stimulation) also delivers electric current however it faces some resistance in penetrating skin and hence it results in an unpleasant feeling.

Interferential Therapy on the other hand penetrates skin more easily and is more comfortable and pleasant than TENS. The two currents produced by Interferential interfere with each other to create signals stronger than TENS which can penetrate deeply to interfere with pain sources. Hence, it can be argued that interferential is one step beyond TENS.

The resulting impulses stimulate underlying tissues and nerves. They are not painful though. Patients feel them as minor prickles on their skin.

How does it work?
The electrotherapy device involves placing electrodes on skin near the affected area. The electrical device generates minute electric impulses which are transmitted through your skin. The crossing over of two or more electric currents cancels parts of electric currents which create a different net current applied to the target tissue.

These low-frequency signals stimulate endorphins, natural painkillers produced by our body. Interferential therapy can treat muscle strains, sprains and spasms.

Many times a physical therapist might use warm moist heat also with the interferential treatment. It involves applying heat to reduce pain and promote healing once swelling has minimized. Heat applied to the affected are attracts blood which is conductor of electricity. More blood in the area create an electrically conducive environment. This enhances the ability of IFC current to stimulate underlying sensory nerves.

Interferential therapy can be used for:

Osteoarthritis, Muscle Reeducation, Acute Edema, Circulatory stimulation and Urinary and Fecal Incontinency.

Falling, Balance and Physical Therapy


Your risk of falling increases with age. About a third of people over 65 and about half of people over 80 will fall once this year. There are lots of causes for this. Vision problems are one common culprit, as are other types of medical conditions, including low blood pressure, neurologic conditions, or muscle weakness. Sometimes, the reasons for falls are much more direct, as is the case with hazards in your home. Nevertheless, even in those kinds of instances, the consequences of falls can be devastating. Physical therapists can help you deal with those consequences as well as reduce your risk of falling in the first place.

One thing your physical therapist will do for you is help you determine, and then reduce, your risk of any further falls. A vision test is part of this process, as is a home safety assessment. A general review of the rest of your medical history will also be performed, including a look at what medications you're currently on. If you're on more than four, that alone can increase your risk of falling. Once this assessment has been performed, a personalized program of therapy will be put together for you. A physical therapist with experience treating people with neurological conditions will be particularly adept at this.

Balance training will be a part of that program. Single leg standing is one simple exercise that can help you improve your balance. Walking and moving in general form another key component. Of course, it won't just be any old walking and moving. Dance steps, figure eights, walking in circles, and even obstacle courses could all be utilized. Next, up, we have "dual task" training. This is learning to do more than one thing at the same time safely. The goal here is that you will be able to maintain your walking speed while engaging in other tasks, such as carrying groceries or engaging in a conversation.

You don't just need to be able to balance yourself while you walk, though. You also need to build up your strength. So, your therapist will also guide you through exercises designed to help you do just that. Not all of your therapy needs to be administered solo, either. There are also several community programs your therapist may encourage you to be involved in. In addition to helping you reduce your risk (and fear) of falling, these programs will provide you many wonderful opportunities for additional socialization and support.

The Importance of Assessment in Massage Therapy


I have a little jar of "blog topic ideas" sitting on my desk, relatively neglected until now. I find it funny that this title was the first one I pulled out because assessment is one of the fundamental parts of my treatment, and the treatments of other health care professionals. Why else would there be so many types of specialists but to look more closely at the details to answer that constant question.... Why?

Assessment is the back bone of a reasonable and rational treatment. In school, I'd suspected this would be true, but, finding assessment difficult, I tried to avoid it; circumstance and ambition would see it otherwise. I attended a seminar given by Rob Johnston DOMTP, principal and one of the head instructors at the Canadian Academy of Osteopathy, here in Hamilton Ontario. It would be impressed upon me that not only is assessment important, it is the only way to truly find and correct the dysfunctions found in the people who come to my office.

So what makes assessment so important? It is the difference between walking into a dark room and crashing into everything to find the one small item you needed, and walking into the same dark room and turning on the light before you begin to search. Massage Therapists have great hands and can feel a great many lumps, bumps, cold spots and textures on the bodies of our clients. But without an assessment, none of that really means anything more than lumps, bumps, cold spots and textures. My first true experience with this was in the seminar I attended. We were asked to palpate (feel) the bony prominences called Transverse Processes of the lumbar vertebrae. Once found our partners were instructed to bend from side to side. It became very obvious that when one bends to one side (say the right), the vertebrae involved in that motion will rotate to the opposite (left). This small bit of information radically changed my treatment. For, that lump I'd been feeling on so many left sides that I dug and dug at to no avail, was a poor little transverse process, right where it should be. This is the power of understanding assessment.

When we assess, we are looking at the expression or symptom, of the dysfunction. This is what makes assessment such a challenge. As assessors, we are interpreting the expression, tracing it back to the root cause in order to discover what is truly at dis-ease. We all know cough medicine doesn't make the sickness go away, it makes the symptoms/expression of it more tolerable. Manual therapy can be very much the same; it can be symptom management, like cough medicine. But unlike cough medicine, when a solid assessment is done, it can identify the root cause of the dis-ease, and, if treatment is within the scope of practice of the practitioner, it can be corrected.

Another reason assessment is so important is investment involved in treatment. Treatment can take a lot of time, and can be costly; more so if the practitioner has a poor idea of what is going on with his client's body. Not knowing why a person has pain means spending a lit of treatment time playing "hit or miss." This is a wasteful use of time for both the practitioner and the client, and a waste of the client's money. Not only is a single treatment spent mostly trying things that likely fail, but the next appointment is another session of trial and error, and the client has lower chances of actually getting better. And note there is a difference between feeling better and getting better. Certainly getting a massage can make that chronic back pain go away for a few days, maybe a week or two, but it probably will be back. So many of my clients want Trigger Point Therapy. They say "oh you can go deeper" or "dig at that!" but all that pressure really does is release natural pain killers, called endorphins. These will last for most of the day, and may even remain for a few days, but these, like cough medicine, are only managing the symptoms. In short time the back pain has returned, and you're once again at the mercy of your therapist and the Pain Scale system.

By doing very simple postural exams, I have learned to identify and correct problems of vertebral alignment, fixation of sacroiliac joints, sciatic nerve compression, rib and thorax immobility, carpal tunnel like syndromes and other joint or limb pains. Every client presents new challenges and new puzzles with all their own unique alignments to investigate. Assessment is an ongoing process, as you change throughout your treatments, and as I change throughout my career, revealing more and more information about your body, and my role in bringing it to the best health it can have. Though it is not always easy, assessment is always necessary. Next time you visit any professional, whether he or she is a Doctor, Nutritionist, Personal Trainer, or Accountant, make sure your investment is protected and respected by having your concern properly assessed, so the prescribed treatment is reasonable and rational.

How to Treat Shoulder Pain


People of all ages suffer from shoulder joint pain. The causes are many and can range from an unknown onset to a traumatic event. Many people develop shoulder pain from performing repetitive type activities for prolonged times. Others will experience pain in the shoulder following a fall on the outstretched arm. Sometimes the reason for pain is not as obvious.

I see many cases where a person cannot identify what caused the pain in the shoulder. Often times this pain can occur when someone bumps their shoulder against the door jam in the middle of the night when going to the bathroom. This rather benign action sets off a chemical process that will build up over several days. Eventually this will cause pain in the shoulder. Since the "injury" occurred several days earlier the person cannot recall any event that started the process. Regardless of the cause of the shoulder joint pain the treatment is often similar from case to case.

Treatment of the shoulder can encompass several different areas. Conservative measures include medication (anti-inflammatories, pain medications), rest, cortisone injections, and/or physical therapy. All of these methods can be effective in helping relieve a painful shoulder especially when they are performed in combination with each other.

Often times when you present to your medical doctor with complaints of shoulder joint pain they will prescribe some medication as described above. Rest will also be advocated although this is not always easy for people to follow depending on their activity levels or their occupations. Injections are sometimes given right away depending on your medical doctor's philosophy. Some MD's would rather you attend physical therapy before considering injecting the painful shoulder. Injections are not the end all beat all and they do not offer relief to all patients that receive them. Often times a well rounded physical therapy program can alleviate the pain and you won't have to consider having an injection.

Physical therapy treatment can consist of the following interventions:

1. Therapeutic exercises - this will consist of range of motion, conditioning, and strengthening exercises. The goal here is to maximize and restore any loss of range of motion, improve the overall endurance of the shoulder blade and shoulder muscles and to improve the strength of these same muscles. The muscles surrounding the shoulder blade must be conditioned and strengthened just as the main shoulder muscles would be because these muscles have to work in conjunction with the shoulder muscles in order to restore normal motion and to prevent recurrence of pain.

2. Manual therapy - this will consist of hands on treatment that the physical therapist would perform on you. This might consist of joint mobilization (i.e. techniques designed to improve joint motion and relieve pain), or soft tissue massage (i.e. techniques designed to improve muscle and connective tissue flexibility and to relieve pain).

3. Modalities - this consists of machines that the physical therapist may apply to your painful shoulder to assist in pain relief and healing along with the other treatments mentioned above. Some of these machines are ultrasound, electrical stimulation, TENS, or iontophoresis.

All of these interventions can be used in the treatment of shoulder joint pain. It is common in clinical practice to use a combination of these interventions. It has been my experience in treating hundreds of shoulder patients over the years that therapeutic exercise is an absolute in the treatment of the painful shoulder. Well rounded exercises that target the shoulder blade and shoulder muscles are the mainstay of any good physical therapy regimen for the treatment of shoulder pain.

The Role of Physical Therapy - Advocacy For Untapped Healing Powers!


What is physical therapy?

Physical therapy [PT or PTs] encompasses the art of healing and empower patients with the courage to cope. Talk about a drug-free experience! Wow! Take that to the bank, surgeons! You could think of PT as, sort of what Jesus did some thousands of years ago when he was on earth; who mind you, is our prime example, because never had he once gave someone a shot of something (drugs or any such contraptions) for any type of sickness presented to him. He may have given them a piece of bread and a fish or two; but, you name it, he can heal: Blindness, blood issues, cripple, dumbness, even death--he simply used his hands and applied His faith in the father. That's it! The rest as we now read it, is history. Cool ha? Sure is!

Although we may never believe, admit nor embrace it's very presence, our profession is faith-based. Think about it? You need to have at least some, to do an effective diagnosis; follow some 18 steps guideline, let alone, manipulate. The lack of it, faith, would lead us to an erroneous chase of endless symptoms; which we do know is the manifestation of something gone wrong. Hence, our primary reason for not treating pain..."not about to happen, but something has already happened" (S.V. Paris). It takes courage to cope with all that life throws at us, and our patients. Many times, all it takes is a good listener. While doctors mean well, and most do, they don't have the extra time to spend with each patient, but PTs do!

Benefits and power of listening:

Good PTs do a full and thorough examination; thus, reducing the possibilities of error, unnecessary surgery, and or strain on the health care system. A successful PT is one who has acute listening skills and highly sensitive (the ability to see through his hands) touch. Most often than any, you will find that, your patients will tell you how to treat them. They will say something like, "it hurts here, there, under, and over there." With exceptional listening skills and attention to details, the clinician should discover a diagnostic lead.

Our role as listener, is to use our God-given gifts, and apply our learned knowledge to make the right diagnosis; which, then leads to the most effective and "if necessary," predictions to bring about a successful change, the outcome. It's amazing that we seldom treat people; on the contrary, we help them treat themselves by providing the necessary information that is pertinent to limiting their dysfunctions, improve restoration and return to normal function.

So what do PTs really do?

They do not prescribe drugs or medications, however, they seek to improve, restore, encourage, motivate, educate, and emancipate patients from their state of dysfunction to a higher level of awareness, both mentally, physically and to change habits that will preserve proper body alignment through postural education, and research-based evidence that will bridge the gap to the lay person's understanding and adaptation to correct body mechanics, right techniques, and self empowerment. That' Physical Therapy! not some "do-one-more-stretch," Mr. Bob, then c'mon back next week, as some would have you believe. We do more than just therapy, we restore!

The main purpose is true restoration:

Noticed that, Jesus' prime reason for coming was to restore us back to the father. His journey also lead him to restoring us back to our normal function, as result of sin and the manifestation of aberrant diseases. We don't have the ability to cure anything in life, only, we restore normal function and lives. Moreover, even though we have not yet tapped into our full potential, could you imagined if we did? Jesus' said to his disciples (at the miracle of the fig tree), "I tell you the truth, marvel not over these simple things (paraphrased) because greater works than these shall you do" (John 14:12).
