ABMP's "Certified" members must have 500 hours of massage education from an approved program (or equivalent) to join and 16 hours of continuing education every two years. The International Massage Association (IMA) originated in 1994 in Washington DC. Today over 70,000 people have joined the IMA Group, Inc. Shiatsu - A Timeless and Effective Massage Therapy Technique


In a massage school in Maui , Hawaii , a group of students are gathered around their teacher as he performs the steps in shiatsu massage. Cyber School Teaches Independence


Cyber school has really helped me a lot as a person.

In public school I had some independence, but if I asked for anything or wanted something it was usually handed to me on a silver platter or right down the hall.

Now that I'm in cyber school I've learned to become a lot more independent and excited about the day. Mainly it's only my family monitoring me now, and telling me to get work done. This means that I can easily slack off or just get by as I sort of did in the beginning of cyber school. If I don't slack off, then I have to find motivation myself.

This change in dynamics has caused me to start being more organized, and thoughtful of my time. The best part is this I have not changed because I have to and that's all I know, but because I want to be successful!

I even find myself at night excited for the day to come, and planning fun and challenging things. For example I've been taking driving really slowly, and last night I decided that I would go shopping by myself. It may not seem very crazy to you, but to me that was a huge milestone and makes my day something that I can be proud of.

Sure it was tough in the beginning to get used to all of the new freedom, and stay motivated. Finally, though, it feels like I've hit a good point where I have full, successful days, and many fun things going on without tiring myself out or getting too stressed out.

I'm also a Christian and my faith and spirituality has really been very healthy lately. I feel like I can finally make my own decisions and grow because I've had time to find out who I really am without hurtful words, false information and strong personalities surrounding me all the time.

Now I am looking into my forward, and contemplating serious and very big questions that will greatly affect my future. It's interesting for me to think that if I were still in public school I'd probably be much more focused on homework or class the next day instead of the future.

Overall I am very happy about my decision to do cyber school because of the independence it has taught me! That is something that I will be able to carry with me through out my entire life.

You've finally mustered up the courage to pursue your passion. You've enrolled in massage therapy school. Now what? It's time to buckle down, learn as much as possible, and of course make good grades. Depending on what your previous educational experiences have been the path to success in massage therapy school may not be evident. Below are a few helpful tips:

Attend Class - The importance of regular class attendance cannot be overemphasized. When you miss classes, you miss lectures, notes, class discussions, homework explanations, and assignments. Remember learning happens in the classroom and studying and reviewing at home.

Be effective in Class - Use your class time wisely. Showing up for class is only the first step in making good grades. Your goal should be to learn as much as possible. Remember this is your career we're talking about. Do every assignment. Learn how to adapt to different instructors and styles of teaching. Sit in the front of the class whenever possible. Be on time to each class and participate. Your instructors want for your success. Communicate your needs and aspirations to them.

Manage your time well- Being organized and making time to study and complete assignments outside of class is essential. Proper time management will ensure that you make the most of your massage school experience. Set aside time to study and don't allow yourself to be interrupted.

Study Smart- Getting started on your studying is often the hardest part. Don't put your studying off until the last minute. This will only lead to stress and frustration. Find a quite place to study with minimal distractions or temptations. Understand your learning style and adjust how you study accordingly. Visual learners learn best from films, pictures, reading, and demonstrations. Auditory learners learn best from lectures, discussions, and videos. If you're a kinesthetic learner, you learn best when you're actively involved in the learning process. Move around while you study, use objects whenever possible, and role play ideas and concepts.

When taking multiple-choice test try to come up with the answer before you look at the choices. Then read all of the answer choices. If you're not sure of the answer cross off the ones you sure are incorrect. If necessary make an educated guess based on the remaining answers. When in doubt go with your instincts.

Good Documentation is Crucial For Maximum Reimbursement


The purpose of documentation is to provide a record of services rendered to support medical necessity, including that services are skilled, rehabilitative, safe and effective. Documentation provides factual information for administrative, regulatory, billing and payment as well as for clinical purposes.

Therapists need to document support that therapy services are needed due to:
o The functional deficits with which the patient presents, i.e., decreased Activities of Daily Living (ADLs) functions
o The impairments identified that cause the functional deficits, e.g., decreased ROM, decreased flexibility, etc. - the Problem List

Documentation to support that the patient would benefit from therapy services in order to:
o Return to previous level or near previous level of function
o Prevent further deterioration or decreased risk factors
o To provide evidence that progress is made during the treatment as relates to both the functional deficits (ADL measurable progress) and measurable impairments (increases in ROM, etc.)

Documentation must support that billing is appropriate by using the appropriate coding per payor and for Medicare, the 8 minutes rule and documentation of Total Times

The Initial Evaluation and Plan of Care demonstrate Medical Necessity and need for Skilled Therapy by:
o Describing the onset of the illness, disease, or exacerbation of a chronic condition
o Painting a picture of the patient's baseline condition - the Prior Level of Function with specific ADLs, e.g., the patient was able to ambulate for 60 minutes to grocery shop
o Documenting the patient's Current Level of Functions with specific ADLS, e.g., the patient is unable to ambulate for more than 5 minutes, a neighbor grocery shops for the patient
o Providing information regarding the patient's living environment, i.e., does the patient live alone? does the patient need assistance for ADLs? are there stairs in home?
o Providing objective measurements to support the need for skilled care. The use of standardized tests should be scored to establish and support a baseline for function and uses to evaluate the patient.

A consistent flowing SOAP note is SO important when it comes to proving medical necessity.

Subjective:
o Age of patient - can be a complexity
o Date of onset - can be a complexity. Use date of last exacerbation or time that patient saw physician for referral, not "insidious."
o Current complaint & history of the complaint
o Pain - include the description, type, intensity - use a pain scale
o Prior Level of Function (PLOF) - use Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
o Current Level of function (CLOF) - use ADLs/IADLs, include use of assistive devices
o Aggravating and ameliorating factors
o Medical History/Medications - both can contribute to complexities
o Complexities/Co-morbidities - document all that might impact the course of the treatment. (This could impact medical necessity for extending treatment beyond the CAP.) Include specificity regarding the severity of the complexity/co- morbidity.
o Precautions - also document on the treatment log at the top in red for continuity of care
o Prior Therapy and the outcome
o Living arrangements - e.g., Live alone? Need assistance from others? Caregiver? Stairs in/out of home? Steps in/out of home? Handrails present-what side?

Objective:
o Measurements
o Tests - use standardized tests whenever possible for functional assessment, e.g., Patient Specific Functional Status and impairments, Tinetti
o Observations - i.e. gait, skin integrity, neurological status, cognitive status, how the patient:
o Sits - posture
o Transfers from sit to stand
o Balances
o Ambulates - distance ambulated, devices used
o Gait - comparison of stance phase and stride length, cadence, ascend and descend stairs
o How patient takes off coat or sweater

Assessment: clinical judgment/subjective impressions

o Determination if treatment is or is not necessary

Plan of Care:

GOALS:

Goals should be:
o Consistent with the identified impairments and the previous functional level of the patient
o Clearly identified both for the Short Term Goals (STGs) and the Long Term Goals (LTGs), e.g., LTG 1.,2.,3. and STG 1.a, 2.a, 3.a or LTG 1.,2.,3. and STG 1., 2., 3.
o Measurable, i.e., # of feet, minutes, amount of assistance needed, # of pounds
o Suggest a STG - Initiate HEP and a LTG - Finalize HEP
o Suggest using STG - documents progress
Examples:
o STG 1 in 2 weeks: Pt will be able to stand at the sink to prepare a small meal
o LTG 1 in 4 weeks: Pt will be able to stand in the kitchen for 30 minutes to prepare a large meal
o STG 1 in 2 weeks: Pt will be able to reach the first shelf in the kitchen to remove a cup
o LTG 1 in 4 weeks: Pt will be able to reach the second shelf in the kitchen to remove a plate
o STG 1 in 2 weeks: Pt will be able to grasp and lift a cup to drink
o LTG: in 4 weeks: Pt will be able to grasp a 1-2 lb pan and lift to stove

Document all of the possible modalities you may use. "Modalities as needed" is not adequate.
Review the FI/MAC/Carrier local coverage determinations (LCDs).

Explain why you chose to use the various treatments/modalities.

As treatment progresses, document in the daily notes and ProgressReports, Evaluations/POC:
o Treatment modifications: document why you are discontinuing and changing to the new exercise/modality
o Your focus of the treatment when using the same exercise for different reasons, e.g., one day the exercise is focused on neuro re-education and the next, the same exercise is focused on increasing ROM
o Major changes to the treatment plan require physician/NPP Re-Certification.

Ask yourself:
o Does my documentation show that the treatment provided could only be given by a licensed Therapist?
o Am I reassessing the progress toward the identified goals?
o Was the treatment maintenance in nature?
o Does my documentation establish a distance baseline of function and clearly show how my patient is responding to the treatment provided and is progressing?
o Am I treating for pain without documentation regarding the severity of the pain and use of a pain scale?

Will The Nintendo Wii Change Physical Therapy?


Many patients begin a painful physical therapy process following shoulder surgery. Following the invasive process of implanting bone anchors or the cutting and replacement of tendons the arm is further inhibited by weeks of immobilization caused by wearing a sling. Once the sling is removed, typically after two to six weeks, the arm is stiff, weak, and painful to move.

The physical therapy process starts with passive stretching to try to regain the range of motion the shoulder joint is capable of. Lack of movement has caused a buildup of scar tissue and a lack of sinovial fluid to "grease" the joint. Initial range of motion exercises are painful and take a great deal of effort. Range of motion returns slowly. Passive, or assisted, movement returns much quicker than active, or patient directed, motion.

After about four weeks the patient begins strengthening exercises. These are also tedious, painful and take a lot of time. The mental focus needed to perform these exercises leave the patient mentally and physically exhausted. The slow pace of recovery often takes its toll on the emotional health of the patient. Around week six or seven, many patients report to onset of mild depression. While sounding extreme, the pattern has repeated itself among members of sports medicine community web forums.

The majority of patients find out that real recovery, the gain of strength and flexibility, occur when the shoulder is used on a day to day basis to do real tasks. A return to exercise is difficult but the activity builds active range of motion very effectively.

The Wii is starting to find a home in hospitals around the country. Herrin hospital in southern Illinois has started to use the video game system and reports that patients refocus their energies into beating the opponent instead of focusing on the pain. This pushes them further into rehab than they would normally. The patients often do much better.

Having spent time competing with friends to see who is the better boxer, I can say that my shoulders were heavily fatigued following play. Whether prolonged use will turn into stronger shoulders or not, I'm not sure. Play definitely got the blood flowing and the muscles working.

For liability purposes Nintendo does not market the Wii has a physical therapy device, although clearly the potential is there. It's only a matter of time before we see systems that use this technology geared primarily for physical therapy. I would not recommend that patients fresh from the sling start swinging virtual racquets and don virtual boxing gloves right away. It's clear that the potential is there for a faster recovery. Definitely talk to your doctor before starting any physical therapy and perhaps the Nintendo Wii is in your future.

Chicago Massage Therapists Unable to Thwart Misguided Massage Therapy Regulation


Illinois is like most other places in the United States, with the government helping to regulate and ensure the professional quality of the massage therapists who live within its jurisdiction. Indeed, 40 of 50 states now have statewide regulations of their massage therapy industries, whose nationwide, annual income totals between $16 and $20 billion. It is therefore surprising and disappointing that the city recently decided to ban the opening of any new Chicago massage therapy businesses unless they have special permits and locate themselves in "industrial corridors".

Fortunately, massage therapy businesses already licensed in the city of Chicago were "grandfathered in" and do not have to move from their current locations. But it seems unfair that new businesses, whose therapists must operate under strict regulation by the state of Illinois with specific professional requirements, must relegate themselves to neighborhoods that feature used-car lots, taverns, liquor stores, and warehouses.

One of the more frustrating aspects of this new ordinance, which officially passed in June of this year, is that Chicago therapists rallied to protest it when the ordinance first came up for a vote in April. Mainly due to this pressure from the industry, the ordinance was tabled in April. But it eventually passed, without much fanfare or notice, despite the objections of the Chicago massage therapy industry and individual Chicago-based therapists.

What does all this mean? Unfortunately, on a philosophical level, it seems to mean that people still associate the industry with others that should be tucked away, out of sight and out of mind (thus the ordinance not banning new businesses, but restricting them to certain areas of the city). This is maddeningly inconsistent considering massage therapy's strict regulation by the state of Illinois.

From a practical standpoint, this ordinance is bad for the Chicago massage therapy industry specifically and business generally. Indeed, not only will new Chicago office massage, or any other therapy business be forced to jump through extra hoops and set up shop in potentially undesirable areas, but it will also be difficult for established businesses to change locations, as they would be subject to the new policy if they decided to move. This inconsistent and unfair grouping of a legitimate and regulated industry with other less-desired business will send the wrong message to other companies and industries who may fear similar treatment.?

Low back pain is commonly associated with soft tissue pathology, often with resulting compensatory muscle guarding (spasm). This often involves both the paraspinal and abdominal musculature. Treating Your Thumbs Like Fine China


If you want a long career in massage, it's important to treat your thumbs like fine china.

Can you remember the first time your thumbs cried for mercy while massaging? Could it have been when you gave your first neck and shoulder massage? It probably lasted all of about two minutes before your hands, in particular your thumbs, started screaming, right? What are the last words you usually hear right before the neck and shoulder massage you're receiving from your non-massage therapist husband, wife or significant other abruptly ends?

"Ow! My thumbs!"

When I teach hands-on massage to massage therapists in training, the first thing I notice is the predominant use of their thumbs. It's so painful for me just to watch the few seconds I allow it to go on before I school them. As humans, it's a natural instinct to want to use our thumbs. After all, we've been given the wonderful gift of opposable thumbs, it seems a shame not to use them at every opportunity. Especially when we find a big juicy knot in the back that just begs for a monster thumb to dig into it. Unfortunately, for massage therapists, it's not the best idea ~ not if you're hoping for longevity in the bodywork industry.

I like to refer to my thumbs, and my students' thumbs as "Fine China." The first time I told that to a class, they looked at me as if I was a nut. Well, despite the fact that I am kind of nuts, part of the reason, I'm sure, is that about half of the class didn't know what fine china was, let alone what it had to do with their thumbs. I explained that fine china is very delicate, often expensive, porcelain dinnerware that can be easily broken; and, it's only brought out for special occasions. Of course, certain porcelains can be quite rugged, but I was trying to instill a vision in their heads of something delicate and rare that needed to be protected from damage.

Being new in the game, they didn't believe the amount of damage ~ sometimes permanent ~ massaging with heavy thumb use for three to six hours in a row, several days in a row could do. This is de rigeur behavior of newbies (until, that is we start our massage trades when they think they're going to die halfway through their first hour-long massage). Every new class was the same.

"Alright, I'd say," as I set my stopwatch. "Let's have a little demonstration. When I say go, I want you to tap your forefingers and your thumbs together, like this ~ tap tap tap ~ until I tell you to stop. Ready? Set... Go!"

And so the tapping would begin. A symphony of 25 to 30 pairs of thumbs and fingers ~ tap tap tap tap tap ~ two taps per second. Ah, so many students started out quite confident, almost mocking me in their tapping.

"See?" they'd smirk. Tap tap tap... "I can do this ALL day."

Tap tap tap... One minute down... Tap tap tap... two minutes, then three... Wait!

"Is that grimacing I see?" I'd ask smugly. "Keep tapping! Keep up that pace! Don't stop until I say!" I'd command like a Drill Sergeant in Basic Training.

I have to say, I almost enjoyed proving my point, though I knew the pain they were beginning to feel. I wanted it to hurt, just enough so they'd get it. Finally, at four minutes I'd stop them and they would breathe a collective sigh of relief as they'd begin rubbing their aching thumbs.

It's unbelievable that four minutes of simply tapping two digits together can create searing pain in the thumb joint, muscles and tendons, but it can.

In a poll taken by terrarosa.com.au in 2008, thumb injuries accounted for between 65 and 83% of the 64 massage therapists interviewed.

I find that outrageous, and sad.

My first year of working as a full-time massage therapist in a busy spa, I worked injured. The whole year. I endured horrible pain in my hand, due to "Hook of Hamate" that began in massage school, and was constantly seeking some relief from my colleagues between sessions. My injury was due in part to the fact that we were so busy, partly because some of the rooms were too small to employ proper body mechanics, and mostly because I was not properly cautioned in my training against overusing my thumbs. I massage with wanton abandon, thinking I was invincible. As a result, I suffered needlessly while I learned a valuable lesson.

Thankfully, I was able to make some key changes to my work life that allowed my hand to heal; and of course, I really began working on my body mechanics. It helped to work in a room that was actually big enough to do massage, but that's another story for another day.

Where I taught, my students were going to end up with a $12,500 student loan balance upon graduation. I knew, based on my own experience, if I didn't find a way to drill into their heads just how delicate their hands, but especially their thumbs were, they would never manage to pay those loans off as massage therapists, because they'd most likely burn through their thumbs and/or hands in about two years.

I taught them that their hands were their money makers and they had to be treated like the super stars they were.

They had to be pampered and protected at all costs.

Prior to beginning trades we'd warm up by shaking our hands, singing "Shake your money makers!" It was rousing good fun, but it also taught them the importance of warming up prior to doing any bodywork. The invigorating, blood pulsing life evident in their ends at the end of the warm up reminded them what the focus of the whole exercise was.

In the midst of a massage trade where the class, with partners, would trade massages, I'd break the silence as I'd thrust my thumbs into the air, pointing to the heavens.

"What are these?" I'd bark in my finest Drill Sergeant voice.

"FINE CHINA!" chorused the students, massagers and the groggy massaged alike.

"When do we use them?"

"ONLY ON SPECIAL OCCASIONS!"

"Carry on."

Every massage class I've ever taught, I've mercilessly drilled the importance of protecting the thumbs into my students' heads, over and over and over again, so that they never forget what brings home the bacon, so-to-speak.

I was reminded of the importance of thumb consciousness when I saw a Tweet recently from a massage therapist mentioning how her thumb was healing and she could "probably" return to work in a week, or something like that. I'll admit, I don't know the particulars ~ for all I know she could have broken it playing basketball or falling off a bike ~ but the likelihood it was a job related injury is pretty high. If so, there's an example of a therapist losing work because the work she does has created undo stress on her money maker.

It's what prompted me to write this piece.

As most therapists know, thumbs are delicate. It's not too difficult to stress the muscles and tendons to the point of creating nerve damage. And, you don't even have to massage for a living to develop crippling Carpal Tunnel Syndrome. I've had several clients who've endured numerous surgeries because they "typed," "put small parts together," or "cleaned teeth" for a living. Anything you do repetitively for long periods of time can cause injury, which can sometimes end up being permanent.

I always struggled with my hands because not only was I a busy massage therapist, but I was also performing music regularly, playing guitar and bass, and in my spare time writing (typing) for hours and hours on end. Add to that home improvement projects and gardening and you've got hands that hurt like the devil most of the time.

I supposed that's what made me more conscious of hand safety, and such an advocate for hand, in particular, thumb well-being. For bodyworkers, and massage therapists specifically, your thumbs and your hands are your livelihood and shouldn't be taken for granted that they'll get you all the way to retirement. Treated right, they can take you all the way. Treat them wrong, and they'll take you nowhere.
